Admission Department Policies

Perpetual Help College of Manila is an institution which offers secondary and tertiary education. Students who intend to seek admission to this institution will need to pass the Entrance Examination to be accepted. Students with exceptionalities will be admitted based upon the school’s ability to meet their educational needs. Applicants are admitted and retained on the basis of their ability to meet the academic, moral, and behavioral standards of the institution.

The right to refuse admission to any applicant who does not meet the admissions requirements is reserved for the institution.

    Junior High School (Grade 7-10) Admission Policies

  • A. Criteria For Admission of New Students and Transferees
  • Must be physically fit and with a pleasant disposition.
  • Must pass the Entrance Examination administered by the school.
  • Must not have been subjected to any disciplinary action in his/her previous school.
  • B. Admission/Application Procedure
  • Fill Out Application Form for Junior High School (Submit 1pc. 1 x 1 ID Picture, Bring 1pc. Long Folder and Bring 1pc. Pencil)
  • Take the Entrance Examination administered by the Guidance Department.
  • Fill out a 201 Form and secure a Locator Number from Admissions Staff that will be attached to the examination result.
  • Wait for release of examination results.
  • Proceed to High School Department, 4th floor, ALTA Building for an interview with the High School Principal.
  • High School Principal will discuss and release requirements to applicant.
  • Proceed to Cashier (Accounting Department) at the 2nd Floor, Main Building for payment of Reservation Fee.
  • Applicant will then submit Enrollment Requirements to High School Department.

    Senior High School (Grade 11-12) Admission Policies

  • A. Criteria For Admission of New Students and Transferees
  • Must be physically fit and with a pleasant disposition.
  • Must pass the Entrance Examination administered by the school.
  • Must not have been subjected to any disciplinary action in his/her previous school.
  • B. Admission/Application Procedure
  • Fill Out Application Form for Senior High School (Submit 1pc. 2 x 2 ID Picture, Bring 1pc. Long Folder and Bring 1pc. Black Ballpen)
  • Take the Entrance Examination administered by the Guidance Department.
  • Fill out a 201 Form and secure a Locator Number from Admissions Staff that will be attached to the examination result.
  • Wait for release of examination results and requirements for enrollment.
  • C. Enrollment Requirements (to be submitted to the Admissions Department after the student has passed the Entrance Examination)
  • Fill out Pre-Enrollment Form – Original and 2 Photocopies
  • Entrance Exam Result – Original and 2 Photocopies
  • Form 138 (Report Card) – Original and 2 Photocopies
  • Two (2) pcs. – 2x2 ID Pictures
  • Certificate of Good Moral Character – Original and 2 Photocopies
  • Birth Certificate (from PSA) – Original and 2 Photocopies
  • Form 137 from previous school (Permanent Record) – Original and 2 Photocopies
  • Junior High School Completer Certificate – Original and 2 Photocopies
  • NCAE Result - Original and 2 Photocopies
  • ESC Certificate(for students from Private Schools) – Original and 2 Photocopies
  • Long Brown Envelope – 2 Pieces
  • D. Admission/Application Procedure
  • Proceed to Senior High School Department Office, 2nd Floor, Main Building for advising.
  • Proceed to Registrar’s Office (Window 1), 2nd Floor, Main Building for advising on your section and schedule.
  • Proceed to Assessment Office (located at the opposite side of Registrar’s Office) to have your schedule and fees assessed.
  • Proceed to Cashier (Window C) at the 2nd Floor, Main Building for payment of assessed fees.
  • Go Back to Registrar’s Office to have Registration Form printed.
  • Give your extra copy of your Registration Form to Senior High School Department

    Tertiary / College Admission Policies

  • A. Criteria For Admission of New Students and Transferees
  • Must be at least 5 feet in height (College of Nursing applicants)
  • Must be physically fit and with a pleasant disposition
  • Must pass the Entrance Examination administered by the school
  • Must pass the initial interview conducted by Admissions Department
  • Must not have been subjected to any disciplinary action in his/her previous school.
  • B. Admission/Application Procedure
  • Fill Out Application Form for College (Submit 1pc. 2 x 2 ID Picture, Bring 1pc. Long Folder and Bring 1pc. Pencil)
  • Take the Entrance Examination administered by the Guidance Department
  • Fill out a 201 Form and secure a Locator Number from Admissions Staff that will be attached to the examination result.
  • Wait for release of examination results
  • Submit all requirements to respective colleges (Part C)
  • Wait for advising of subjects and schedule from College Department
  • Proceed to Cashier (Accounting Department) at the 2nd Floor, Main Building for payment of fees.
  • C. Enrollment Requirements (to be submitted to the respective College Department after the student has passed the Entrance Examination)
  • Form 138 (For Freshman applicants only) – 2 Photocopies
  • Transcript of Records or TOR (for Transferee students) – 2 Photocopies
  • Honorable Dismissal (For Transferee students) – 2 Photocopies
  • Certificate of Good Moral Character – 2 Photocopies
  • Evaluation of Academic Credentials (For Transferee students) - 2 Photocopies
  • Chest X-ray Result (College of Nursing, Physical Therapy, Occupational Therapy and Respiratory Applicants only) – 2 Photocopies
  • Birth Certificate (from NSO) – 2 Photocopies
  • Hepa-B Screening Result for applicants of the following: (College of Nursing, College of Physical Therapy/Occupational Therapy/Respiratory Therapy and College of Hospitality Management *formerly HRM) – Original Copy
  • Two (2) pieces – 2x2 ID Pictures
  • Two (2) pieces – Long Brown Envelope
  • Two (2) pieces – Long Plastic Envelope
  • One (1) piece – Long Folder
  • Form 137 (For Transferee students) – get request letter from PHCM Registrar’s Office
  • Fully accomplished Student Personal Information Sheet (SPIS)

Incomplete Requirements

Enrollees with incomplete requirements will be required to make a promissory note or letter to the School Registrar’s Office. An indicated deadline of the submission of incomplete requirements will be written in the promissory note or letter.

Admission of students who failed the Entrance Exam

When an applicant fails the Entrance Exam given by the Guidance Office, Admission’s Department will be informed about the situation of the applicant so that Admissions Department can advice the applicant about his/her situation.

Scholarship

Scholarships are open for the Top#1-10 Grade 12 graduating Senior High School students who are applying for College Freshman (he/she must be able to prove that he/she belongs to the Top #1-10 students in his/her batch). Details will be discussed in Admissions Office.

Any scholarship grant will be evaluated first by the Office of Student Affairs and Guidance Office. Then, it will be reviewed by management.
